Role Purpose

Social Work Case Manager Summary

The Social Work Case Manager is responsible for providing support to individuals and families in need. The Case Manager will evaluate client needs, develop and implement treatment plans, and coordinate services to meet individual and family needs. This position requires strong organizational and communication skills, as well as the ability to work with diverse clients in a variety of settings. Social Work Case Manager Duties
